Job Description for Targeted Case Management

SUMMARY OF RESPONSIBLITIES

The Community Support Specialist is responsible for the comprehensive services offered through Targeted Case Management program. This includes assessment, case management, psycho education, crisis intervention, advocacy, and community liaison. The Community Support Specialist is also responsible for preparation of client records for internal and external audits. The Community Support Specialist is responsible for the submission of treatment plans for authorizations as well as maintenance of charts. The Community Support Specialist is responsible for all administrative tasks as well as clinical supervision of each employee. The Community Support Specialist must be able to work independently and be proficient in resource development. She/he is responsible for the development, implementation and adherence to program policy.   In addition, the Community Support Specialist is responsible for the training of new employees and the supervision of the program. A Community Support Specialist is responsible for maintaining their level of skill by completing the requirements of their licensure and/or required agency and COMAR training. In addition, each employee is expected to abide by all safety rules and regulations of the department and facility. The Community Support Specialist is responsible for carrying out the mission, philosophy of care and policies of APS LLC. She/he also performs other duties as needed to achieve program goals and objectives.

Supervision and Coordination of Services

The Community Support Specialist reports directly to their immediate supervisor and/or the Community Support Specialist Supervisor. The Community Support Specialist coordinates with other professionals within the agency as well as outside agencies to create an interdisciplinary team approach as well as other assigned duties. The employee abides by the ethics set forth by the agency and the profession as a whole.  

DESIRABLE MINIMUM QUALIFICATIONS

Knowledge, Skills, and Abilities: The Community Support Specialist should have either experience or education related to the population with whom they shall work. The employee should have knowledge of the DSM and relevant symptoms of the most common diagnosis within the TCM programs. The Community Support Specialist will be able to demonstrate proficiency in assessments, treatment planning, advocacy skills and case management skills. The employee demonstrates the ability to express themselves well through writing, interpersonal and oral communication skills. She/he will demonstrate an understanding and sensitivity to serving culturally diverse and/or special needs population while maintaining client confidentially.

Education and Experience:
Bachelors degree in a mental health related field and one year of mental health experience or a Bachelors degree in an unrelated field and two years of mental health experience.  

SALARY:
Based on the current ranges established by Human Resources Department and level of experience.
