Job Description for Rehabilitation Counselor-Adult

(The Adult PRP Counselor is an instrumental part of the client’s care team and provides a variety of services and supports designed to foster independence, understanding, and recovery. All Adult staff are trained in mental health first aid and are provided with tools and resources needed to encourage self-actualization in their clients. The Adult PRP Counselor is a dynamic, personable, and determined person with the ability to manage time effectively in a variety of office and community-based environments)

Classification
Non-exempt, hourly, part time or full time.

Salary Grade
Based on the current ranges established by Human Resources department, experience, and related education.  

Reports to Rehabilitation Specialist and/or the Program Director.

Job Description and Summary/Objective

The Rehabilitation Counselor is responsible for the comprehensive services offered through on-site and off-site programs. This includes assessment, case management, behavioral support services, psycho education, crisis intervention, advocacy, and community liaison. The Rehabilitation Counselor is also responsible for the timely and accurate completion of all documentation such as treatment plans and progress notes. Staff is responsible for assessing and teaching community skills including Activities of Daily Living as needed.  Counselor will submit all necessary documentation in accordance to COMAR regulations and promptly according to agency guidelines.  Rehabilitation Counselor is responsible for maintaining their level of skill by completing the required 8 hours of training per year above and beyond the required basic COMAR trainings. In addition, each employee is expected to abide by all safety rules and regulations of the department and facility. The Rehabilitation Counselor is responsible for carrying out the mission, philosophy of care and policies of APS LLC.  She/he also performs other duties as needed to achieve program goals and objectives.

Essential Functions
*Timely and accurately complete clinical documentation.
*Facilitate on-site group activities and community outings as assigned.
*Conducts in-home services with clients and serves as a community liaison between the client and various organizations. *Offsite programs visit clients in-home on a daily basis; Onsite programs visit clients in-home as needed.
*
Serves food as needed in compliance with Maryland food safety regulations.
*Communicating with team members and outside providers for coordination of care.
*Managing a caseload of roughly 15-20 clients for Offsite programs and roughly 20-22 clients for Onsite programs. *Caseload sizes may vary dependent upon number of services typically provided to the clients on an average basis.  
*
Assess client needs and develop Individualized Rehabilitation Plans according to their goals.
*Engage clients in services and provide comprehensive care which may include but is not limited to the following:
*Educating clients about community resourcesEncouraging clients to perform daily living activities
*Assisting clients with benefit and resource acquisition and maintenance
*Promote independent living skills, self-care skills, and social skills
*Provides transportation for clients as needed in accordance with clients’ rehabilitation goals.


Competencies
1. Writing, interpersonal and oral communication skills
2. Knowledge of the DSM and relevant symptoms of the most common diagnosis within the PRP programs
3. Proficiency in assessments, treatment planning, advocacy skills, and case management skills
4. Basic knowledge of Microsoft Office Programs
5. Demonstrate an understanding and sensitivity to serving culturally diverse and/or special needs population
6. Patient confidentiality
7. Operation of a 15 passenger van
8. Time management

Supervisory Responsibility

Rehabilitation counselors do not possess supervisory responsibilities.

Work Environment

This job operates in a professional office environment requiring routine use of standard office equipment as well as out in the community.

Physical Demands

In order to perform the duties of this job, the employee is regularly required to communicate and hear. The employee is required to stand, walk, use hands and fingers, and reach with hands and arms. The employee is also required to regularly operate a motor vehicle.

Position Type/Expected Hours of Work

To be determined based on assigned program and type of employment.

Supervision and Coordination of Services:  

The Rehabilitation Counselor will report directly to their immediate supervisor (the Rehabilitation Specialist), and/or the Program Director. The Rehabilitation Counselor will coordinate with other agency employees, as well as with multiple outside organizations and service providers in order to create an interdisciplinary team approach. The employee abides by the ethics set forth by the agency and the profession as a whole.

Travel

Position requires employee to travel to and from client homes and to other service providers as needed according to their clients’ individualized goals as well as to and from the various locations of the organization as needed for trainings, meetings, and other events as necessary. Employee utilizes the company vehicles, including a 15 passenger van, and their personal vehicle.

Required Education and Experience

1.) High school diploma.
2.) 3-4 years of experience working with individuals with chronic mental health issues (may be substituted with further education)

Preferred Education and Experience
1.) Bachelor’s degree in Human Services related field from an accredited institution and/or equivalent experience.
2.) 1-year experience in mental health setting.  

Special Requirements (to be acquired within the first 90 days of employment)
1. Valid Driver’s License (must have at time of hire)
2. Privileging and Credentialing
3. DLA-20 Certification
4. CPR Certification
