Adult Psychiatric Rehabilitation Program (PRP)


Adult PRP services are available to those individuals who are 18 years of age and older, who are receiving mental health services from a physician or licensed clinician, who have or are eligible for medical assistance*, and who meet criteria for a priority population mental health diagnosis**.  Services are offered in multiple settings/combination of settings in order to best meet the needs of the client (i.e. onsite day program, offsite outings, in-home, etc.).  Following a period of assessment which evaluates client strengths and preferences, individualized rehabilitation goals are implemented. 


Target areas may include but are not limited to the following:

  • Development of natural supports through increased opportunities for socialization 

  • Connection to community resources

  • Navigation of the entitlement system (i.e. SSA, DSS, local housing agencies)/Case management

  • Advocacy

  • Life skills training (i.e. grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication)

  • Development of crisis management and wellness self-management/recovery related skills

  • Assistance with scheduling and maintaining appointments

  • Coordination of care with other providers/treatment team members (i.e. therapist, psychiatrists, PCP, substance abuse providers, specialists)

  • Mobility skills training (i.e. assistance with applying for various forms of reduced cost transportation, assistance with understanding the bus system, etc.)

  • Other areas of need as identified


*Individuals who do not have medical assistance may be eligible for PRP services under the following conditions: Individual has been ordered by the Court to follow treatment recommendations which include PRP services; individual is transitioning out of a higher level of care such as RRP, individual has received inpatient psychiatric care within the past 90 days,  or the individual is already receiving PRP services through another provider.
